Greener Pastures for Seniors collaborates with health care agencies to provide physician in home visits to Guilford and Forsyth county residents that have age related and other disabilities that confine them to their home.

"Confined to the home" means that one is home bound and travel outside the home is difficult and taxing to the point special transportation may be required for medical treatment.                              - More Info

How Does It Work?       A Physician or Nurse Practitioner will come to the home of the individual to assess the medical needs. Based on this assessment, it is determined whether or not the person is eligible. If eligible, then primary in-home care appointments can be scheduled. Appointments can be arranged for most primary care needs, which include routine lab work, injections and yearly exams.   When an appointment is made, a Physician or Nurse Practitioner will come to the home. This medical professional is specialized to meet the various needs of older adults.

What Agencies are Involved?   Local healthcare agencies partner with Greener Pastures for Seniors to provide this unique service.  The healthcare agencies provide the specialized medical staff and medical treatment, while Greener Pastures for Seniors funds capital expenditures, marketing and advertising expenses.  This is a community-based program and open to all that have disabilities that confine them to the home. What  is the Cost? There is no upfront cost for the medical professional traveling to the home. Just like any other office visit, insurance will be filed and the client will be billed a small amount for the co-pay.
