Thomas M. Whyte, M.D., FAAFP provides in home care to patients in the South Triad regions.  Dr. Whyte maintains an active office medical practice, and has extensive experience in providing care in the hospital, nursing home, and home settings.
Dr. Whyte brings high quality medical evaluation to patients confined to home by medical or mental disabilities.

Family and Community Medicine is honored to provide quality, patient-centered healthcare to the home bound since 2001. Their practice mission is to improve the quality of life for those who can not seek care outside the home.

Home bound Services include:

· Evaluation and treatment of adult patients who are home bound due to physical and mental disabilities.

· Evaluation and management of multiple chronic health conditions, such as diabetes, high blood pressure, heart disease, stroke, arthritis, dementia, etc.

· Assessment and assistance in obtaining admission to an Assisted Living Facility or Nursing home when necessary.

· Coordination of care with other home health agencies such as Home Health Nurses, CAP Program, Physical Therapy, Speech Therapy, Social Services or Hospice.

Appointments can be scheduled by calling the office of Family and Community Medicine at (336) 672-3200. Patients are seen Monday-Friday, 8 a.m. – 5 p.m.
